Introduction: The aim of this study was to determine whether surgical tactics leading to a reduction in tumour involved surgical margins translates into improved local control.

Methods: Retrospective review of a consecutive case series (n=162), of previously untreated patients, undergoing surgery for oral cavity or oropharynx squamous cell carcinoma (SCC). Extensive use was made of CT multiplanar imaging to plan primary tumour resections.

Results: A total of 9 patients (5.6%) had a tumour at the resection margin. The local control rate at 36 months was 96.3%, disease specific survival (DSS) was 85.7% and overall survival (OS) 76.5%.

Conclusions: Primary surgery for oral cavity and oropharynx SCC, carefully planned to minimise tumour involved margins, combined with conventional adjuvant treatment where indicated, is associated with a high probability of local control and disease specific survival.
