Introduction:  Acinic cell carcinoma (ACC) is the rarest form of low-grade malignant salivary neoplasm in which the predominant site is the parotid gland. It is usually more frequently diagnosed in women compared to men. Surgical excision is the main treatment of this malignant neoplasm.

Material and methods: A 46- years old Malay lady presented with right parotid swelling of a year duration, insidious in onset and progressive in nature. Head and neck examination revealed a firm to hard multilobulated swelling over right parotid gland which was tender measuring 3 cm x 3 cm. Facial nerve function was intact. There were no cervical swellings noted. Other examinations were unremarkable.  

Results:  A pre-operative computed tomography (CT) of the neck showed a well- defined right intra parotid mass involving the superficial lobe measuring 5 cm x  4 cm x 1.5 cm. She then underwent superficial parotidectomy with preservation of facial nerve. Intraoperatively, the mass was within the superficial lobe, it appeared multilobulated, well-encapsulated and  measured 4cm x 3 cm x 2 cm. The deep lobe of the parotid gland appeared normal. Post-operative recovery was uneventful. Histopathological examination reported as Acinic Cell Carcinoma of the parotid with surgical margins clear. She was referred to Oncologist and planned for radiotherapy.

Conclusion:   Acinic cell carcinoma is a low grade malignant salivary neoplasm which is rarely diagnosed. Careful long term follow up protocol is advised in view of its malignant potential. It requires a high index of clinical suspicion, prompt diagnosis and definitive initial surgical treatment. If the condition is appropriately dealt with from the beginning, its morbidity is extremely low and permanent cure is highly probable.
